Both have their unique features as wells as drawbacks. Kitchen carts are great if you are looking for a larger piece of kitchen furniture that will double as an additional space for preparing meals, as well as drawer and possibly cabinet space.

Having a kitchen cart is much like having a nice little table that you can store things in and that looks nice just sitting in the middle of the kitchen. A butcher block island is a little different, it too can have some sort of storage space, but are normally a bit smaller than the kitchen cart.

The main different is that a buffet is usually put against a wall instead of in the middle of the kitchen. All of these pieces can be drawn together to make any kitchen appear to be extraordinary.
